    Making a claim for medical expenses

    Claim documents should be submitted within 30 days of treatment or discharge from a hospital, medical center or clinic. Original receipts must be submitted (not photocopies).

    You'll need to submit the following documents:

    1. A completed claim form
    2. A medical report stating your symptoms, diagnosis and treatment. For treatment of skin diseases, the name of the prescription is also required.
    3. Original receipts containing breakdown of costs
    4. A copy of your ID card
    5. A copy of your membership card


    ​*Important: We will not pay your claim if you're not able to provide an original receipt for your medical costs.

    You can indicate how you would like to receive your claim reimbursement on your claim form or the 'Change Request Form'

    We offer 2 reimbursement methods:

    • bank transfer
    • by cheque

    Important: ​Any changes to your claim reimbursement method will only be made if consent by the policy holder (individual or employee) is obtained in writing, via completion of a 'Change Request Form'.

    ​Processing time
    If your claim is eligible, we will pay your claim within 15 days of us receiving all documents required to process your claim.

    The Claims Department will take up to 15 days to process and pay your claim (provided that you have complete evidence supporting your claim and Allianz Ayudhya can consider providing coverage without the need to verify any additional information).

    However, in the event that the insured wishes for the company to reconsider the claim result, the company will complete the review within 30 days from the request date for review or receive additional evidence (if any).
